Complete the following form to nominate a service dog for inclusion in a future issue. Please note:

By submitting your nomination, you certify that you are the owner of the photo and text entered, along with all associated rights
By submitting your photo and text, you grant KibbleChat.com, it's editors and management staff, and any assignees, the right to reproduce your photo and text in future editions of the KibbleChat eNewsletter, on the KibbleChat.com website, and on any other promotional materials for the site or newsletter.
You may submit as many nominations as you wish.
